I certify that the information on this application and its supporting documents is accurate and complete. I understand and agree that failure to fully complete the form, or misrepresentation or omission of facts, represents grounds for elimination from consideration for employment, or termination after employment if discovered at a later date. I authorize Buffalo Biodiesel Inc. to investigate, without liability, all statements contained in this application and supporting materials. I authorize references and former employers, without liability, to make full response to any inquiries in connection with this application for employment. I agree to submit to a physical exam, criminal background investigation, and/or screening for illegal substances upon conditional offer of employment. I understand that this document is NOT an offer of employment, and that an offer of employment, if tendered, does NOT constitute a contract for continued guaranteed employment.

I hereby authorize Buffalo Biodiesel, Inc. and/or its agents to make an independent investigation of my background, references, character, past employment, education, adult criminal or police records, and motor vehicle records including those maintained by both public and private organizations and all public records for the purpose of confirming the information contained on my Application and/or obtaining other information which may be material to my qualifications for service now and, if applicable, during the tenure of my employment or service with Buffalo Biodiesel, Inc. and/or its agents.

I release Buffalo Biodiesel, Inc., and/or its agents and any person or entity, which provides information pursuant to this authorization, from any and all liabilities, claims or law suits in regards to the information obtained from any and all of the above referenced sources used. The following is my true and complete legal name and all information is true and correct to the best of my knowledge:

Federal Drivers Privacy Protection Act Authorization to Obtain in Motor Vehicle Records
Pursuant to 18 USC §2721, et seq. Applicant/Employee Authorization and Potential Employer/Employer Authorization

I authorize Lawley Service Inc and Its Affiliated Branches to obtain my Motor Vehicle Record ("MVR") and release the information to my employer/prospective employer Buffalo Biodiesel Inc. I understand that this MVR may contain personal information relating to any driving restriction (i.e., "must wear corrective lenses") in addition to any/all driver violations and/or accidents which may be on record through The State Department of Motor Vehicles.

In addition, should my application be accepted for employment and/or upon my becoming an employee for Buffalo Biodiesel Inc. or as a condition of being assigned driving duties on behalf of the aforementioned, I further authorize any/all additional request for my motor Vehicle Report be submitted and reviewed as needed for the purpose of and the following permissible uses, pursuant to 18 USC §2721:

(a) My continued evaluation and eligibility standards under the State and Federal regulatory compliance standards;
(b) For use by any insurer or insurance support organization, or by a self insured entity, or its agents, employees, or contractors, in connection with claims investigation services, anti-fraud activities, rating or underwriting; and/or
(c) For use by an employer or its agent or insurer to obtain or verify information relating to a holder of a commercial driver's license that is required under Chapter 313 of Title 49 (49 USC §31301, et seq.).

Employee/Applicant may obtain a copy of his/her MVR by contacting Lawley Agency, LLC at 361 Delaware Avenue, Buffalo, NY, 14202 at 716-849-8618.

I hereby authorize Buffalo Biodiesel Inc. and/or its agents to test for controlled substances and alcohol by agreeing to provide a urine sample or blood sample to a certified drug testing supervisor and/or at the designated medical provider assigned by Buffalo Biodiesel Inc. and/or any of its agents. Results may be relevant to my qualification for employment now and, if applicable, during the tenure of my employment with Buffalo Biodiesel and/or its agents.

I release Buffalo Biodiesel and/or its agents and any person or entity, which provides the results pursuant to this authorization, from any and all liabilities, claims, or lawsuits in regards to the result information obtained from any of the above referenced sources.

I understand that refusal to submit to drug testing will be deemed a failure and will result in my termination of employment.

I agree to submit to immediate post accident drug/alcohol testing, AFTER I receive medical treatment (if any is required) that involves property damage or personal injury while on duty. Failure to submit to post accident drug/alcohol testing will be deemed a failure and I understand it will result in my termination of employment.
